Upstream, our prevention program and our partnership with P\u00c9TAL help find solutions to prevent homelessness whenever possible. Access to permanent housing does not mean the need for support ends. Flexible support, adapted to each woman's reality and pace, remains essential to sustaining long-term stability.  <br><br><b>Offered for up to 18 months<\/b>, this personalized support includes help with moving and settling into the new home, ongoing psychosocial follow-up at varying levels of intensity, support in consolidating gains made (coping skills, social ties, day-to-day independence), and referrals to community and institutional resources in the new living
